EARLY INTERVENTION SERVICE MATRIX
Listed below you will find the services provided through the Ontario County Early Intervention Program. Upon referral to our program, an Initial Service Coordinator will call you to set up an appointment to visit you and your child. They will be responsible for explaining the Early Intervention Program to you and assisting you in the steps necessary to determine if your child is in need of any services.
If an evaluation is requested, you have the opportunity to choose one of twelve approved Evaluators. If the evaluation determines your child eligible for services there will then be an IFSP (Individual Family Service Plan) meeting scheduled to identify the family and child's needs and the desired outcomes from services. This meeting will consist of the family, the evaluator, the initial service coordinator and the county EIO (Early Intervention Official).
The child's best interest is utmost important and the family will have a very active role in the decisions made at this meeting. At this meeting a written plan will be completed and agreed upon specifying the services required, the frequency of the services, the providers of the services and the location of services; this is the IFSP.
The IFSP will be reviewed every six months in order to add, modify or delete any goals/objectives, strategies and/or services. If a parent requests, the IFSP may be reviewed earlier than six months. Once the child is eligible to transition, he/she will transition to the age 3-5 Preschool Program, which is coordinated through their school district, if they are still determined eligible for services.
