The Home Health Care Agency is a Medicare Certified Home Health Agency providing quality home care services to Ontario County residents.

Skilled Nursing Services:
Includes assessment, teaching, case management, and treatments and procedures such as wound care, and intravenous therapy.

Rehabilitative Services:
Includes Physical Therapy, Speech Therapy, Occupational Therapy, Medical Social Worker Services and Nutrition.

Home Health Aides:
Are available to assist with personal care, meal preparation, and other activities of daily living.

Home Care Coordination:
Provided by Home Health Nurses at the local hospitals in conjunction with the social work departments. This coordinated approach allows for a smooth transition from hospital to home.

PRI/Screen
Patient Review Instrument. This is an assessment required by New York State to determine eligibility for those seeking nursing home placement.
